Effective September 7, 2021, all in-person adapted students are required to confirm daily that they have completed and passed the COVID-19 school screening. All students must screen for COVID-19 before going to school each day. Students are to use the Ontario Government's online screening tool:
Attestation: I VERIFY THAT I HAVE COMPLETED AND PASSED THE COVID-19 SCHOOL SCREENING ONLINE TOOL FOR TODAY. This screening cannot diagnose you. If you have medical questions, consult a healthcare provider or your local public health unit. Listen to the advice of your local public health unit first, as their advice overrules the advice in this screening.
Complete the MTS Student Daily Attestation Form to verify that you have completed the self-screening.
