Return to School LDCSB Parent/Guardian Form

Every family MUST complete a Return to School Form for each child attending a LDCSB elementary and/or secondary school before August 20th, 2020. Before you begin, please ensure you have reviewed our Safe Return to School Plan at https://www.ldcsb.ca/apps/pages/index.jsp?uREC_ID=1076300&type=d&pREC_ID=1373661 and have your child(ren)’s Pupil Number (student login number) or OEN (shown on report cards). New to the LDCSB or cannot find the the Pupil Number or OEN? Please call the Board Office at 519.663.2088 or email [email protected]. Due to the volume of calls, you may have to leave a message.
